Causes of sagging breast tissue?

Ageing and drooping breasts can be common amongst women. Some women struggle to find clothing, bras and swimsuits that fit their sagging breasts. Drooping or sagging breasts can be caused by a number of factors including pregnancy, breastfeeding, weight fluctuations, ageing, and gravity.

There are three medical conditions caused by sagging or drooping breasts. These are:

  • Ptosis where the breasts sag and the nipples point downward, past the inframammary crease – the crease beneath the breast. This is caused by breastfeeding, weight loss and ageing.
  • Empty Sac Syndrome where the breast has lost fullness however the nipple and areola have remained in their normal place.
  • Pseudoptosis which is similar to Empty Sac Syndrome however the breast lacks fullness above the nipple, rather than below it.

Breast Implants (Mammoplasty)

Many patients ask if they need breast implants with their breast lift.  Some women who have good skin elasticity and sufficient breast tissue only require a breast lift (mastopexy). However, women who have decreased breast tissue or who are looking to increase the size of their breasts have experience a better outcome with Breast Lift (Mastopexy) with Implants (Mammoplasty).

The suitability of your breast implant size, shape and type will depend on your consultation with your surgeon, your body type, your degree of ptosis, your desired shape and your desired size.  Working in collaboration with Dr Drielsma will help ensure that your desired results are met.

You can also have corrective nipple and areola surgery to adjust the position of your nipples, if needed, at the same time as your breast lift (mastopexy) surgery. If you suffer from inverted nipples, disproportionate nipples, or nipple hypertrophy this will also be adjusted during your surgery.

What Type of Implants?

When considering Breast Lift (Mastopexy) with Implants (Mammoplasty), it is important to discuss the best type of implant for your body, desired size, and desired shape. There are a number of implants available in different sizes, shapes, and fillers.

Saline Filled Implants

Saline implants have a “water bed” feel and have been known to cause a “ripple effect” on the surface of the breast. This can occur when the patient has small amounts of breast tissue to begin with and the ripple of the saline within the implant can be seen on the skin surface.

This may be reduced by using smooth surfaced, round implants that are overfilled – within manufacturer specifications – resulting in an implant that is more firm to the touch. This can make the implant look more spherical or round, rather than a more natural teardrop shape.

However, if saline implants deflate, they do so quickly and the saline is easily absorbed by the body.

Silicone Gel Filled Implants

Silicone Implants are designed to have a more natural feel, similar to that of breast tissue. They are found to hold their shape better than saline implants and tend not to deflate.  However, the accepted rupture rate for silicone implants is around 10% over 10 years, which will result in the need for removal or replacement of the implants.

There is also an increased risk of capsule contracture or hardening of the breast with silicone implants – again around 10% over 10 years.

Smooth Surfaced Implants

Smooth implants have a softer feel and are easier to insert. They most closely mimic the feel of real breast tissue.

Round Implants

Round implants are less natural looking than tear drop or anatomically shaped implants. These implants may give a better lift than naturally shaped implants or to create contrast between the upper and lower parts of the breast. If a round implant rotates in the breast pocket, it doesn’t tend to affect the breast shape.

Tear Drop or Anatomical Implants

Anatomical implants tend to be more versatile than round implants. The tear drop shape gives a more natural look to the breast and often better results depending on your desired outcome. However, if the implant rotates in the breast pocket, it needs to be repositioned to recover the natural shape of the breast.

What Size Implants?

Choosing a post-operative cup size doesn’t always mean you’ll fit exactly into that size; however, in general the goal is to augment your breasts to fit your body size, your skin elasticity, your lifestyle, and your desired look.

Cleavage is achievable with Breast Lift (Mastopexy) with Implants (Mammoplasty) procedures. The ultimate outcome will depend on your expectations of surgery and your consultation with Dr Drielsma.

How is a Breast Lift (Mastopexy) with Implants (Mammoplasty) Procedure Performed?

The breast is lifted by removing excess breast skin and tightening the remaining skin.  In the case where the breast has also lost volume and tissue, an implant is added to aid the overall look of the breast lift (mastopexy).  The breast implant (mammoplasty) helps to avoid an empty or deflated look.  In most cases the implant is placed beneath your pectoral muscle as placing the implant above the muscle can accelerate ptosis rather than relieve it.

How Much Downtime is there with a Breast Lift (Mastopexy) with Implants (Mammoplasty) surgery?

Your procedure will take around 3 to 3.5  hours, though this can differ depending on your body, desired results and the positioning of your implants. You will be able to return to work after 2 weeks and return to normal physical activity within 6 weeks.

There will be visible bruising that should fade within 2 weeks and swelling that will begin to subside after 3 weeks. Keep in mind that it can take 12 months for your breasts to finally settle into their final shape, size and position. Do not return to vigorous exercise until you are cleared by Dr. Drielsma and ensure you always wear a supportive bra while exercising.

Are There Pre-Operative Requirements?

You will require a referral  from your GP to book your consultation.   You should stop smoking preferably 2 weeks before surgery and stop any aspirin or other NSAIDs (non-steroidal anti-inflammatory drugs) at least one week before surgery to avoid blood thinning complications.

What Happens During Recovery?

You are given specific instructions to follow during your post-operative recovery period. You are likely to experience swelling and discomfort for 3-4 weeks and should avoid strenuous activity or heavy lifting for 6 weeks. Pain management will be discussed and customised to suit your individual needs.  This will be discussed and organised during your consultations with Dr Drielsma.

Are There Risks?

There are risks with any surgical procedure and as such there are some potential risks and complications when it comes to plastic surgery.  Complications are uncommon but they can include infections, unfavourable scars, seroma (the accumulation of fluid), wrinkling, leaking or rupture of the implant, capsular contracture, and asymmetry. Common side effects of breast lift with implants surgery are bruising and swelling.
